In short

Personomics—why chronic disease and “doing everything right” often still fails, and how personalized functional medicine uses at-home lab testing to identify deficiencies/toxicities and tailor protocols.

Guest backgrounds

Dr. Stephen Cabral, board-certified naturopathic doctor, best-selling author, founder of Equilife; specializes in personalized functional medicine, at-home lab testing, data-driven wellness; 300,000+ client appointments worldwide.

Key claims

Conventional medicine is great for acute emergencies but often can’t address chronic issues (brain fog, low mood, weight gain, inflammation). Same diagnosis can require different protocols because underlying causes differ (e.g., gut dysfunction, deficiencies, toxicity, stress/hormone patterns). The body “heals itself” once supported; don’t guess—test.

Notable examples

Hypothyroidism case studies—Sam vs Joe: iodine deficiency, low iron, gut dysfunction/SIBO/candida/intestinal permeability leading to autoimmune thyroid attack. Weight-loss example: cutting calories/exercising more can lower metabolism and cause rebound. At-home testing examples: heavy metals (e.g., mercury), cortisol, food sensitivities; sauna/cold plunge may be inappropriate depending on cortisol/anxiety/electrolytes/histamine balance.

Visit highlevelfire.com. highlevelfire.com. Dr. Cabral, say what's up to Fire Nation and share something that you believe about becoming successful that most people disagree with. Great to be here with you today. Appreciate you having me on. And I would say the thing, at least in my field, which is natural health, is that doing what everyone else is doing or the latest trend, that it's going to get you the same results as the online guru or maybe even a coworker that you're looking to get those same results.

1:58Unfortunately, that's not how the human body works. And we're going to talk a lot about personalization and what works for one person isn't necessarily going to work for another.

2:05John Lee Dumas:Yeah. And I love that. That's actually why I love having Dr. Cabral as my personal functional medicine doctor, because down here in Palmas Del Mar, I mean, everybody's a biohacker. So there's always a person being like, this is the best thing ever. It's that thing. And I can just be like, let me run it through my guy. Cause it may be great for you, but he knows me. He knows me with the countless number of tests that we've done over the years. And Fire Nation, we're talking about personomics, the end of all disease. And I want to first start off by talking about what modern healthcare is actually missing.

2:34John Lee Dumas:So let's get specific here, Dr. Cabral. What problem in today's healthcare system were you trying to solve when you wrote personomics, the end of all disease? Well, I always have to give that disclaimer that conventional medicine is amazing. The technology is amazing. The breakthroughs are amazing in acute-based care. So if you're having a heart attack or your blood pressure is 200 over 140, yes, you should absolutely go to emergency care and you'll be treated by some of the most brilliant physicians in the world. However, most people aren't dealing with that at this moment. They're dealing with chronic-based disease.

3:10They're dealing with brain fog, low mood, weight gain, inflammation, hearing starting to go. They feel like they're aging. And there is no cure for that with conventional medicine. They have to wait until standard blood work enables you to basically fall out of range, which is why for me for 10 years, conventional medicine really couldn't figure out what was wrong with me because I wasn't out of range enough. And so what happens is if they can't prescribe a specific pharmaceutical drug for a disease based state, there's nothing they can do. They simply weren't trained in healthy lifestyle. A lot of that is healthy biohacking, whether it's nutrition or sleep or exercise and movement and how much for what person.

3:50So personomics is really the study of you. It's what you need as an individual in order to rebalance your body. And that'll be different for each individual. It could be a missing vitamin or mineral or enzyme. It might be an issue with your omega-6s to omega-3s. It might be high cortisol, low thyroid. But all of that is easily discernible now. And fortunately, all of us have access to that literally all around the world.

4:14John Lee Dumas:I want to talk about when doing everything right still doesn't work because that can be so frustrating to people. I mean, everybody I talk to, I mean, they claim at least they're following the rules. They're eating clean, they're exercising, they're taking their quote unquote supplements, you know, whatever that may be. You still don't feel well. So why does this happen? And what does personomics reveal about why effort alone is not enough? A big part of this is, we should use some real examples. So when we think about, there's so many people who want to lose some weight or they at least want to lose maybe five or 10 pounds of body fat.

4:48And so when we think about this, we can say, okay, well, the typical mantra is going to be, you should eat less, which typically means drop your calories. Maybe you're following a specific macro plan and you should move your body more. And I've been doing this for 26 years. My team and I have seen over 300 ,000 one-on-one client appointments. We've run well over a quarter million of labs. And I can tell you that there are many people who come to us and all around the world, this isn't just a US-based thing. And they say, I am doing everything right. I'm following the nutrition. I'm lowered my calories.

5:19I'm exercising five days a week, and yet I still can't keep the weight off. And if I do anything outside of my diet and exercise routine, I gain back two or three pounds in a week. And the issue then isn't necessarily discipline. It's not working harder. If you drop your calories more and you exercise more, you're simply going to lower your metabolism more, which means then in the future, if you can't maintain this crazy diet and exercise routine, you're going to gain more weight back that much easier because you've lowered your metabolic rate. And so my goal for people is to understand that once you get past the foundation, so yes, to lose weight, you should eat a healthy, nutritious diet.

5:56And we could talk about what that is and anti-inflammatory, et cetera. And you should move your body. How much is based on your body. And also after you've done the foundation, there are other levels to ascend to, right? Maslow's hierarchy of needs in terms of psychology. Well, there's also the same for the body. Once you've mastered nutrition and movement, Now we need to look at sleep, hormone balance, stress levels, toxicity, and much more. So what my goal is, is to give people an actual personalized plan that goes beyond the basics. The basics are important. That's your entry level. It's like Curves, Curves Gym.

6:28It's an eight machine circuit back in the day. Great place to start, but you don't want to be there two to three years after you started there. You want to graduate to the next program.

6:35John Lee Dumas:And that's where I want to move on to next, which is the same diagnosis, but different protocols. How specifically does Personomics explain why two people, let's just call them person A and person B. No, let's call them Sam and Joe. Why could Sam and Joe have the exact same diagnosis but require two completely different treatment approaches? How does that make sense? Exactly. And in Personomics, I talk about this with literally five different case studies, all of them diagnosed by conventional medicine with low thyroid or hypothyroidism. And so if we just even use two of those examples, one person can say, again, I'm doing everything right in order to improve my thyroid.

7:17Why isn't it balanced? Why is my TSH still above a 2.5? And so when we look at this, we have to say, okay, what deficiency might this person have or what toxicity a person might have? So some of those deficiencies could be Sam has low levels of iodine. He's just not getting a lot of iodine in his diet. If you don't get iodine, you don't make thyroid hormone. It's literally made out of iodine. Another one could be, especially for women and during their years of having a cycle, menstrual cycle, they could have low iron. It's one of the biggest reasons for low levels of thyroid. Another one could be higher levels of gut dysfunction.

7:55So instead of just a deficiency such as zinc or selenium or iodine or iron that we just spoke about, all of those could cause low thyroid. Another one could be intestinal permeability. And luckily now conventional medicine is even starting to talk more about this. If there's candida overgrowth or yeast overgrowth or parasites or SIBO, which is small intestinal bacterial overgrowth, and the gut becomes more permeable. Remember, there's about 26 feet of intestinal tract. If that becomes permeable, what happens is we start to get proteins and bacteria spilling out into the bloodstream. As they say, 70%, 80 % of your immune system is in that gut associated lymphoid tissue, the GALT.

8:30Once you start to produce more and more immune-based cells and these bacteria move throughout the body, it can create an autoimmune-based process. This is a toxicity-based, and now those immune cells can begin to attack the thyroid. Part of this is genetic, but remember, genetic needs the right environment, right? So it's not just our genes. When I was 17, I had rheumatoid arthritis. I had Addison's disease. I had type 2 diabetes. I had POTS. I had mass activation syndrome. I had acid reflux, all sorts of issues, and I don't anymore. Well, I didn't have them really before 17. And I had them through 27, but I didn't have them after 27.

9:04And it's been 20 years. So the reason is same genes, different environment. And so what we look to do is change the environment for the individual where disease can't exist in a healthy body.

9:15John Lee Dumas:Fire Nation, this is powerful stuff. And I think you can really understand, I mean, what modern healthcare is missing, why doing everything right still doesn't work sometimes, and how the same diagnosis can really still require two different protocols for reasons that were just talked about. And we have a lot more to talk about, about learning the body's language, ownership through at-home testing, and more when we get back from thanking our sponsors. 12:26John Lee Dumas:Stop piecing tools together. Start building momentum. Visit highlevelfire.com and start building your something today. Dr. Cabral, we're back, and I want to talk about learning the body's language. I mean, what are the key signals the body gives when something is just kind of like out of balance? And how can people begin to read those signals instead of ignoring them or what seems to happen a lot to suppressing them? And even before you get diagnosed with, let's say, a disease because your blood works out of range, your body will give you those signals. Oftentimes, though, we do try to suppress them, whether it's with over the corner medications or maybe prescription for these symptoms.

13:06And so I like to look at the questions that I like to ask are, you know, what is going on with your digestion? Do you get bloating? Do you have gas, constipation, loose stool? Do you feel like your stomach's a lead weight after you eat? All of these things are important. So there's digestion, there's energy. Do you wake up with energy or do you need caffeine in order to start your day? I have no issue with people having a cup of coffee after breakfast. There's a lot of polyphenols, all those great things. And if their HPA axis or their adrenals can handle it, no problem at all. Like I have no issue with that.

13:35However, if you don't have natural energy, we need to ask why. And there's always an underlying root cause. And no, it has nothing to do with age alone. The next one we look at is mood. Are you irritable? Are you overwhelmed? Do you lack ambition and drive and really that vitality for life? Because if you don't have that, again, it's not an aging issue because who's to say when that should disappear? Is it you disappear just at 47 years old? Or is it 65? Is it 82? Because I can show you individuals of all those different ages with energy. So why them and not you? And the last two are actually with sleep, which is a big one.

14:12And so if you're awake, maybe you fall asleep fine, but you wake up three or four times throughout the night or you wake up at two o 'clock and you have a difficult time going back to bed. All of these have underlying root causes. All of them can be fixed. And the last one that a lot of people are paying attention to as they get older is their skin. And that's just more outward appearance. Are there more fine lines and wrinkles? Is the hair grain faster than it used to? These are usually signs that there's more oxidative stress in the body. There's more free radicals. And so although we can't stop the aging process, we can dramatically slow it down once we find out two things.

14:47What are your deficiencies and what are your toxicities? I know it sounds kind of basic in that way, but that is how you help bring the body and the mind back into balance.

14:55John Lee Dumas:I actually think it's good that it sounds basic because, I mean, then people could understand it. And if they understand it, they can say, okay, I can see why this will help. I can see why this will work. And this kind of moves us into taking ownership through at-home testing. So how specifically does at-home integrative health lab tests, how does that change the way people engage with their health in your experience? So even for myself, when I was 17 through 19 years old, I went to over about two dozen different medical specialists in Boston, Massachusetts. Brilliant doctors. My PCP went to Harvard.

15:28Brilliant guy. The issue is they were only running blood work. and all they found off of my blood work were really low white blood cells. And so they discerned that it wasn't cancer. And once they found that, they had no idea what else it was. So I was left guessing going from specialist to specialist, being told that it's either all in my head or they could see all my glands were swollen, my lymph nodes, but they didn't know why. And so it would have to get worse in my blood work before they could do something about it. And me being, you know, a stubborn, persistent individual, I said there has to be another way, but this is back in the mid to late 90s.

16:01And so I was just fortunate to be able to find some natural health practitioners that did what we now do and like as they call it, pay it forward to share it with others. And that was run these functional medicine lab tests. So I ran my cortisol levels. I ran my food sensitivities. I ran my heavy metals and I had high levels of mercury. I had candida overgrowth. I had SIBO. I had all sorts of different imbalances, low levels of magnesium, low levels of B6, low levels of zinc. And so what I was able to do was finally for the first time to stop guessing because I was taking like 30 different supplements.

16:34I didn't know which one would work, but I was like, oh, well, if I take all these that I've read about, maybe that'll be helpful. And unfortunately, some put too much in my body of what I didn't need. And some of them I was just missing. I didn't get enough of them. So with at-home lab testing, and again, this is for ages three years old and up, this has been around for over 40 plus years used by NASA, FBI. They're all CLIA certified. They're held to the same standard, but they look at gut health and they look at hormone levels throughout the day that you just can't do with blood work. They'll look at your heavy metals and just so much more.

17:06So it's invaluable data. So instead of guessing, we test. Because literally, if you're like, well, I think I should add cold plunging. I think I should add sauna. I think I should add this. I can tell you why. Yes, those are great for some people. And why not? They're not great for others. If you have high levels of anxiety, high levels of cortisol, low thyroid, you don't want to be cold plunging, right? Without a doubt. For sauna, sauna is amazing. I recommend it to nine out of 10 people. But for the one out of 10, I don't because they are already burnt out. They have low levels of electrolytes.

17:35They have very poor or high levels of histamine-based balance. And so for them, right now, sauna is not gonna be appropriate. In four to six months, appropriate. Just not right now. And that's what at-home lab testing allows you to do.

17:47John Lee Dumas:So let's talk about going from guessing to personalization. I mean, this has kind of been a theme of our conversation here today. And if somebody's listening, and let's be honest, It happens, especially when it comes to health and wellness. I mean, they're overwhelmed by conflicting health advice. I mean, you know, their sister's a nurse, their mother-in-law's a doctor. I mean, they're just getting everything from everywhere. What would you say is the very first action that they should take to stop guessing and start personalizing their health? And we have people come in at all different levels.

18:18So some people want to just literally dive in headfirst. There are other people who want to dip their toe in. And there's, of course, people in between. mean, if you want to go all in, you run something called the big five. You've run the big five before. And I can only say that because you've already mentioned it before, of course. And that gives you, that essentially looks at everything. Your fruit sensitivities, heavy metals, gut-based function, vitamin levels, mineral levels, inflammation levels from omega-6s to omega-3s, all your hormones, metabolic function, mitochondria, and so much more.

18:46And again, it's just simple hair sample, urine sample, saliva sample, and a blood spot card. So no needles, no blood draw needed. And the nice thing is you do it in the privacy of your home. It's not shared with health insurance, not shared with your doctor. You can choose to if you want. And it's just with your practitioner at the lab, they give you a full 360 plan of your health. That's all in. But there's also dipping your toe. Dipping your toe would be the minerals and metals test. It's the easiest one to run. As I said, ages three years old and up, a couple of snips of hair, and the data is invaluable.

19:17You literally get anywhere from the last 30 to 90 days of data. Your hair is a protein. It's the same way they measure all the minerals in food. You burn it, turn into an ash, and you look at what has actually been excreted. And so what we're looking for there is higher levels or lower levels of electrolyte-based stress. We're looking at mineral levels like zinc and copper that we need for thyroid and energy in the immune system. We look at phosphorus in terms of either high bone or potential bone breakdown, or we're looking at maybe too lower protein in the diet. We're looking at your heavy metals, lead and arsenic and cadmium and aluminum and mercury.

19:56All of those we don't want in your body. If it's excreted in your hair, it was in your body. It's good that it's being removed. But of course, what is the exposure? Where is it coming from? Is it the mercury amalgams in your teeth? Is it from maybe high levels of tuna consumption? Is it from the water? And so these are all questions we ask. And we simply help people return that body to a state of balance because we know that if we replace your deficiencies and we help you to remove your toxicities, your body heals on its own. A natural health practitioner, I'm a board certified doctor of naturopathy, but you can work with integrative health practitioners.

20:28You can work with a lot of people who run these labs. But when you do this, it allows your body to have the raw material that it needs to heal itself. We don't heal anybody. The body heals itself. It's just the body needs some support in this toxic world that we live in, where there's literally now over 120 ,000 man-made chemicals in the environment, most of which have never been tested against each other. All of them have been tested short-term in small amounts, but none have really been tested over 20, 30 years in our lifespan and in combination. So the goal is go all in or dip your toe in with the minerals and metals test.

21:07John Lee Dumas:Personomics, the end of all disease. What does Fire Nation you need to know about this book? Where do they get it? Share it all. Personomics.io book is a nice little spot and go there. It'll just send you to Amazon, but it allows you to get the free audio book. Since there are many people who like to listen versus read, I totally get that. It's good to get them booked though as well. And the reason I say that is that it has all the self quizzes in there. So even if you can't run labs, it allows you to self-assess yourself, which is really why it's nice to have it as at least a companion guide.

21:40and I'm sorry, I cut you off. What were you gonna say? No, I just said awesome. Excellent, I thought you were gonna say something about audio books because I know you're a big audio book listener.

21:47John Lee Dumas:And Fire Nation has, yeah. Yes, exactly. I love to read, so I'm just a little bit different in that way, old school. I actually like to hold the book. So there's something for everybody. But that really, it gets you started as to understanding that everything is able to be fixed. Everything is able to be healed. But first we need to stop guessing, start testing, look at, blood work is amazing to run. I recommend it for everybody. But if we're not looking at the underlying root cause as to why the blood work may be off, we're not going to be able to ever fix what those imbalances are. And instead, we're going to have to palliate those symptoms through medications as people get older.

22:24And simply, that's not what I want for myself, my family, or anyone in our community, your community as well. So we did put something together for your community, and that's at stephencabral.com slash fire. That's actually half off for your community, that minerals and metals test that I was speaking about. Never any pressure, of course, on our end, but if someone wants to take advantage of it and they've never run a functional medicine test before, it might be a nice place to start.

22:45John Lee Dumas:That's a great place to start. I remember when I ran mine for the first time and came back and was like, all right, we got some work to do. And we did the work and man, it was powerful. Obviously, case on the work as well. And FireNationPersonomics.io book. Check that out for sure.
