2:02And then you see other people that are often not nearly as prominent and as much in the public eye. But they've made those choices to give themselves joyous. Joy-filled lives, they don't ask for anything more. While in some ways I may not be a classically religious person, I talk to God half a dozen times a day. And the thing I say most often is just thank you. Something happens that I recognize that has added to my life. And I can't just resist roll your eyes to heaven and say thank you.

2:40John Lee Dumas:I mean, gratitude. Gratitude is something that so many people go so long without exhibiting any signs of. And it's really sad because we have so much to be grateful for in this world. And I love that perspective, Dr. Jerry. And that's why we're talking today about rejecting normal aging, the longevity blueprints. And you are the man, the myth, the legend behind this. And I want to kind of start all of this by talking about hormone optimization because you focus on balancing and cycling hormones to restore energy, libido, performance. So what does that actually look like for both men and women?

3:18You know, this is where I started with my clinics 30 years ago. So it's, for us, is old news. And I'm surprised that so many of my colleagues are just catching up with parts of it and still haven't really caught on. Hormones are tools that your body uses to do its job. The anabolic hormones, by definition, are those things that allow you to grow lean tissue. And so dehydroepiandosterone, pregnenolone, testosterone, IGF-1, the growth hormone. These are things that allow you to grow new tissue and heal and repair when damage has been done. We've got other things such as pregnenolone, progesterone.

4:00In men and women, one thing I think people miss, Men and women make the same hormones. We just make different amounts of them. So pregnenolone, progesterone, they are anti-stress, relaxation, mellow out, get a good night's sleep hormones. Testosterone, on the other hand, is a stress tolerance hormone. Those super high levels in adolescent males make them think they're bulletproof. they do the most outrageous things to themselves and they think they're tough because it doesn't hurt yeah kid when you get old and everything hurts because your testosterone level is blood you know i mean come on it's easy to be tough when it doesn't hurt right um so we've got a whole host of hormones and we measure the levels we start with at least eight basic hormones in every man and every woman.

4:58And those hormones typically decline with age. The older you get, the lower they get. And the lower they get, the less impact you get from them. You know, for instance, the testosterone and estrogen, people talk about those as sex hormones. Well, they are in adolescent when you're preparing your body for reproduction. but once you're in your 20s they're primarily brain hormones they drive the way we think the way we feel the way we perceive ourselves and the universe around us and as they decline our perception of ourselves and the universe changes and so example women lose 80 to 85 % of their testosterone by age 50 Do you realize 50 % of the women in America by menopause are on a psychotropic?

5:56It's as though my colleagues think that menopause is a Prozac deficiency. And it's not. The other thing that people don't understand is that the primary female hormone is not estrogen. It's testosterone. A healthy 18 or 20-year-old woman has got two to two and a half times more testosterone in her bloodstream than she has estrogen. And so she peaks her testosterone around 15, and that's when her libido peaks. And that's the last time we want them having sex and getting pregnant, and yet that's when their drive is the highest it's going to be in their life. Because their fertility is at its highest at that point.

6:44You know, sometimes on my radio show, guys will call in and say, my wife is 50 years old and she's totally lost interest in sex. What can I do to fix her? And I have to tell them, you can't. There's nothing wrong with her. The purpose of sex biologically is not your pleasure. It's reproduction. At 50, she has no fertile eggs left. If she can't reproduce, why should she have sex? okay her brain may not say that but her body has just said it loud and clear okay now if if i restore all of her hormones to youthful levels okay about 30 of those women will get their libido back

7:25John Lee Dumas:the others won't i mean there's a lot that we can talk about with hormone optimization unfortunately what's going to have to happen is you know our listeners are just going to have to get your book lifelong i mean that's where all the greatness is fire nation because we have a lot to talk about today. And one topic that I'm obsessed with is zombie cells. I mean, they're getting a lot of attention right now. Can you talk about zombie cells? Like what are they and how does clearing them impact both aging and recovery? Every cell in your body is supposed to do its job for a certain period of time.

7:56And it hits the end of its expected lifespan. And it's supposed to undergo something called autophagy. That is literally self-eating. And it's supposed to be destroyed and you make a brand new cell to replace it to carry on the job that the previous one was doing. So skin cells do skin, gut cells do gut, heart cells do heart, etc. A small percentage of cells, hit the end of their expected lifespan, and instead of dying and being replaced, they actually become senile. They change their shape, they change their morphology, their nucleus changes, and instead of doing their job, they kind of become the zombies of the movies.

8:45They start kicking out these anti-growth, anti-healing, anti-repair, pro-cancer, pro-degeneration chemicals into your bloodstream. And they actually recruit healthy cells to become zombie cells. And the more of these you get, the more you get. How do we get rid of them? We do have ways to get rid of them. It turns out the tissue in your body that is most prone to becoming zombified is fat. It's 17 times as likely to become fat as the average tissue in the human body. The second most likely tissue to become zombified is your immune system. And that's important because when you're young, you develop a senescent cell and your healthy young immune system comes in and kills it, destroys it.

9:36So in a young person, that zombie will last less than a day and it'll be gone. But because fat makes zombies 17 times more likely than most of us and the immune system makes it 15 times more likely, what'll happen is at some point, as you get older and you're getting fatter, you add body fat, you rapidly add zombies because that tissue makes more zombies than any other tissue in your body. So they're just accelerating about age 45 to 50, maybe 55, if you're really lucky, you start getting this almost, well, this rapid acceleration in the number of zombies that are being made. But also the immune system that's responsible for destroying them, it has become zombified.

10:27It is saturated with senescent cells.

10:29John Lee Dumas:What are some of the best ways just to get rid of zombie cells? Well, one is the one that is the hardest to do. and that is a you lose weight and b you make lots of muscle and you use that muscle vigorously and intensely and frequently because muscle kicks out over 600 we call them myokines peptides enzymes hormones that not only destroy zombies they help you to grow new brain cells they help your heart to repair they help you to grow new bone they help you to grow new muscle they rescue stem cells throughout your body and help you to have better tissue. The problem is that exercise is hard. What about like seven to 10 day water only fasts?

11:14John Lee Dumas:Does that have any clinical data? The fast will help you lose some fat. And the intermittent fasting actually alters the immune system. And so it helps to rev that up, raises your natural killer cell levels to some degree. But if you really want to get rid of the zombies, You have to use medications that we now know will kill zombie cells selectively. You can ablate them. I've got people in my clinic in their 70s and 80s in which 80 plus, 80 to 85 % of their immune system was now senile. And after three to six months of therapy, they're down to adolescent levels. They're down to four and 5 % that is now senile.

14:27I turned 79 years old. And at 79, I run three miles most mornings. I can bench press over 200 pounds. I married to a woman who is 30 years my junior. My youngest child just turned 14. she was born when I was 65. All right. Compared to when I was 50, when I was fat and round and depressed and sexless, I could not have run around the block if my life depended on it at 50. And now as I'm pushing 80, I run, jump, leap, cavort, dance into the night. All right. So I started with the hormone therapies. We moved all of my hormones. And by the way, we check our hormones in our clinic every three months.

15:16And every three months, we check them all. We check our inflammatory markers. We check our growth factors. We check a whole lot of stuff. And we don't use age-normed values. We use the mid-range of a healthy 25-year-old as our hormonal target for everything. and we move all the hormones in your body back to a 25-year-old range. The result of that is your body has tools to work with. And so what we did was we take that curve where people are sliding downhill and we move them back up the hill. That part was very good, but what we noticed over the last 30 years was that that wasn't enough. Even if we kept their hormone levels at the 25-year-old range, They were still gaining weight.

16:06They were still getting aches and pains. Their joints were starting to wear. They couldn't run as fast. They ran faster than other people their age, but they were still slowing down. And that was where the senescence load started coming in. We started looking at the research to see at what rate were we accumulating these senescent cells and what could we do about it? And then as we started treating people with senolytics and lowering that senescent cell load, all of a sudden we started finding that people were improving their function again. I run much faster now than I did three years ago when I started senescent therapy.

16:51okay and so by getting rid of the senescent cell load that is doing the destruction and inhibiting healing and growth okay reduce the senescent load and include that with the hormone supplementation to provide the tools to heal and repair i mean the biggest thing that

17:09John Lee Dumas:excites me about all of this is just the fact that you know you've figured out that men and women are different. I mean, that sounds obvious, but a lot of people don't take a step back and actually identify that. And obviously from that, you found that aging impacts men and women differently. So how should people think about things like menopause, testosterone, and even retirement when it comes to long-term health? Never become menopausal. We should start intervening with women in their thirties. In fact, in men too. We should not wait until we are old and decrepit and falling apart. We should start when the decline first begins.

17:49And men and women peak our physical capacity about age 25. By 30, we've lost about 1%. The problem is we lose progressively faster and faster as we age. And by the time you're in your 70s, you're plummeting downhill. But let me give you a simple example. We've got papers out there, especially Donovitz and glazier these two doctors gave testosterone supplementation to middle-aged women women in their mid-50s the average age in their studies was 54 when they started and they followed them for 15 years supplementing them with testosterone and what they found was it cut the um it cut the risk

18:37John Lee Dumas:of breast cancer by nearly 85 percent 85 percent and the women that would take the testosterone the entire time they dropped now if they only took it for a few years they got a more modest drop yeah so some women you know some groups some people that only took it for a year or two had a 30 or 40 percent reduction but people who took it consistently if you give women back the the testosterone that is anti-breast cancer remember testosterone is the opposite of breast cancer. Okay. You give them back those youthful levels of breast cancer, their risk of breast cancer goes down. But when does breast cancer really begin?

19:15It doesn't start off as a cancer. What you do is you start with a healthy breast tissue in her teens and twenties. And then at some point, usually in her thirties to early forties, she develops some atypia, some cells that aren't cancerous, but they're just a bit atypical. And then over the next several years, those cells become dysplastic. That's a more severe form of atypia. And so this dysplasia progresses. And then at some point, some of those cells will progress to the point of malignancy. Okay. And then she'll have the cancer three to five years before it gets big enough for us to find it because it starts as one cell.

19:56We can't find one cell. So three to five years later, is big enough. Now we can find it on a mammogram. We say, lady, you got breast cancer. And she thinks, I just got breast cancer today. No, you had it three to five years, lady. But the process started in your 30s. As your estrogen and your testosterone were going down, you were losing the protection that they afforded you. We should be going back in their 30s and never letting those hormones go down. But what we're doing is waiting until the damage gets done and then we're trying to fix it. Why not prevent the damage?

20:34John Lee Dumas:Let's be proactive. Fire Nation, let's be proactive. Absolutely. You know, I started therapy at 50. I wish to God I knew the stuff I knew now at 30. I've done the best I can do. Pretty darn good. Let's be honest. You're almost 79 years old. You're running three miles a day. You're bench pressing over 200 pounds. You look fantastic. You're fathering children. I mean, you were 15 years ago, 14 years ago. My wife has said no more. No mas. No mas. I love it. She's turning 50. Yeah. Fantastic.
