0:00Hey guys, welcome to a very special segment on skin anarchy. I'm very excited to announce that we have been published in the Journal of cellular immunology for our mini review our mini review entitled inflammation and aging the skin inflammasome in the context of longevity science this mini review is very special to me because i am really starting to dive deeper into the world of longevity medicine there has been so much buzz in the media about longevity and just what it means i know a lot of you have seen things by individuals like dr sinclair you know and then there's all these leading experts out there that are really voicing their opinions about this about their medical opinions, whether they're MDs, PhDs, physician scientists, everyone's really kind of chiming in right now because I think the frontier truly is longevity.

0:49Let's be honest. All of us, I think, innately know that. I mean, look around at how many of you know somebody that's on an NAD supplement already. You know, NAD supplements just became a thing. The science just got validated and here we all are already translating it to real products and real innovations that can benefit our daily lives. And so this is an incredibly exciting time for medicine, science, all of those things. So I want to actually dive into the review. When you guys check it out, you'll see what I mean. But what I try to do in this review is kind of bring all that science together that's out right now.

1:25It's floating around. There's been very like kind of preliminary level papers published about it. I'm not going to lie. Longevity medicine is an ocean, but we are far from understanding, You know, just like we don't know much about the actual ocean, we don't know much about longevity medicine. You know, it's very, very similar in that way. So whatever is out there, we've tried our best to compile that information in a very short, bite-sized amount so that you get a hit of everything. And it creates this wonderful landscape for you to understand, like, this is what's going on, this is what we found out, here's where we are right now.

1:56That's the focus of this review. So let's dive into some of the sections. One of the biggest sections, and I think one of the most important, is the beginning. And that is really defining inflammation, defining what does inflammatory aging mean? What does the idea of chronic inflammation really imply when it comes to things like skin health, tissue health, barrier support, anything like that? I think it gives you an innate kind of mental note taken that I now know what this means. And then we dive in further. So then we went ahead and we started talking about things like, hey, cellular senescence is a huge topic.

2:31It's coming up. And, you know, what have we discovered so far? NAD decline. I know we just talked about that. Why does NAD decline? When does it decline? We also talked about one of the best parts of this review is that we haven't put anything except just facts here. So a lot of the things that are mentioned are things like actual pathways and actual targets, like molecular targets of therapy. For example, one of the big, big pathways discussed and molecular mechanisms discussed is the NLRP3 inflammasome activation pathway. That pathway alone, it has two steps to it. So it's one is like priming the cells to respond.

3:10And the other step is activation of the actual cells so that when they do respond. So basically your body saying it's getting the cells ready to do their thing and get very pro-inflammatory. So that's kind of like, you know, defining what that is so that it can be understood that this is how cells work. You know, your body doesn't just one day decide, like, I'm just going to be inflamed. I'm just going to flare up. No, it's like a whole thing that happens, a very complex set of pathways that kick in. And the NLRP3 mechanism is very central to understanding that and understanding inflammation on this level.

3:43So we've talked about that. But we've also talked about acne and understanding the pathophysiology of acne, how that ties into inflammation and the inflammasome. So as I told you guys earlier, there's the step called priming acne. What it does is that it actually in the long haul primes your cells so that they will be more readily activated. So something like that has been discussed here. I think that's very important to discuss. We've also talked about other diseases such as atopic dermatitis, psoriasis, hyperpigmentation, which I know a lot of us deal with. understanding the key markers there, what leads to the true pathologies behind these different things and understanding that.

4:21And then also, you know, we went ahead from that base and then went deeper into the big buzzy things that are around right now with longevity. So we talked about NAD, but what about like sirtuins? Sirtuins have been also talked about in the cosmetic industry, in the skincare industry. I know of a few products that actually are working around the science of sirtuins, you know, and their understanding of how they work in all of this, you know. So we've discussed that. We've also talked about the concept of drug repurposing. Now, I know a lot of you have a friend or a family member that's on drugs like metformin, for example.

4:57Recently, there has been research done that metformin might have longevity benefits. So we've discussed that here. We've talked about drugs like statins. Also, the more controversial ones like the mTOR inhibitors such as rapamycin which I know they became a little buzzy when there's some pretty prominent personalities in the longevity space and I think rapamycin is one of those drugs that's been experimented with by individuals such as that and it's really been discussed on other podcasts like I think Joe Rogan had a segment on this as well but you know we've talked about that here right in this review that that's one of the repurposed drugs that's coming up and so we've discussed a lot of other things as well such as the leaky gut syndrome, what that means for inflammation, how that really contributes to everything, any kind of psycho neuroendocrine pathways, dietary influences.

5:46For example, with dietary, I don't think we understand that anti-inflammatory nutrients are not very different from the ones we are always told about. So one of the big components is understanding that much of what's being discussed in the longevity space is kind of repurposing the science we already know. and so that's a theme that is recurring throughout when you dive into the longevity research that is currently available right now public access it's very repetitive in that way where there's not too much novel science here you know it's more of understanding like hey we always knew about this but guess what we just we're rediscovering it right and we're looking at it now from a whole different lens than we were back then for that whatever pathology we're studying it for.

6:32So that's really a recurring theme and you'll see that in this mini review that we've talked a lot about those things and if any of you out there do have the science backgrounds that can make some of the things we've discussed a little bit more familiar to you, you'll see that right away that oh okay I've yeah of course we know about certain cytokines like IL-17, we know about what NAD is, we understand what AMPK activators are, but here it's all coming together. So another thing that I wanted to point out is that there's also a section on therapeutic strategies and understanding that there are considerable challenges that are emerging because the space is blowing up and it's growing so rapidly.

7:09I love when science takes off and I love when scientists and physicians and everybody in these industries come together to really work towards a common goal which I really believe is what's happening right now in longevity medicine but one of the big downfalls of that is that we often start to implement the things we're discovering a little too quickly and prematurely i've definitely seen that i think for example you know i talked to you guys earlier about rapamycin one of the biggest problems with that is mTOR inhibition which is what it does it's too big of a target to inhibit mTOR it's like basically telling your body hey i'm gonna blunt your immune response so when you get exposed to things that could cause severe infections you're not gonna have an immune response really ready to go with there you know so like that's very important to understand and so that's when things like you know we're now discovering like oh yeah rapamycin great idea but hey can we take that to like microdose levels you know that kind of thing so it's really really crucial i think for all of us to kind of get behind this movement in a meaningful way so that we can ask better questions so that we can avoid mistakes that might occur through things like drug repurposing right so that's kind of the gist of the review i hope you'll check it out read the actual review check out some of the references we've listed we cited over i believe 45 references total which for a very short review that's a really great amount of literature that has been kind of accounted for here so i hope you'll check it out i hope you'll chime in with your ideas and further this discussion i'm definitely going to be putting in some effort from our end to interview more a longevity specialist on the podcast that we can really shine a light on the medicine the science i think there's a lot to unpack here and it's really really interesting stuff once you start to understand how it all works together in this complex system we call the human body so i really hope you'll stay tuned for that but thanks guys and check it out hey guys so i hope you love that episode please make sure to hit subscribe if you're tuning in to us on any podcast platform we are available on so many different platforms so wherever it is that you're tuning in, just go hit subscribe.
