12 tests chem 20 which is highest number of tests ایسا package میں کرے تو rates بہت کم ہوتے ہیں and patients کو بہت فیدہ ہوتا ہے اگر individual test itemize کرے تو it can cost can increase so that's individual 312 312 I think individual test یا نہیں patient کو ایسا بیماری ہوں گا کی pneumonia panel کبھی کہ panel ایک test کا 40 ,000 ہو سکتا ہے okay because it's a panel see it's a technology advance ہو رہا ہے نا راج the problem is that this knowledge and understanding people should have see ایک زمانے میں see 1945 میں ہندوستان کی lifespan کیا تھا 28 years ہم ہمارا lifespan کہاں پر ہے 70 سے اوپر ہے 75 سال تک چلے گیا کئیسا ہوا ہے health improvement جب ہم ٹھیک طرح سے ڈائیگنوز کر رہے ہیں یہ دو تیسٹ کر رہے ہیں 28 years میں worse than any other country not too long not too long ago 70 سال سے پہلے ہم 28 سال میں مر رہے تھے because technology investigations now because of this technologies, diagnostics the cost is higher اگر نہیں کرے اگر hospital ہوا کے مر گیا there is also a problem so lab charges are that then medical consumable charges آپ کو ابھی بتایا تھا نا so medical consumable charges اس میں pharmacy drugs IV fluids injections etc then miscellaneous charges میں کافی طرح food جو you know the items that cannot be classified gloves you know ایسے چیز ہم پورے miscellaneous میں ڈالتے ہیں چائے ڈائیٹ کھانا کھانا you know meals nephro charges کیا ہوتے ہیں نیفرو چارجز وہ کیڈنی کے چارجز ہے یہ نیفرو چارجز ان کے کچھ ڈائیلیسس ہوا ہسپیل میں اب جیسے میڈیکل کنزیومیبل چارجز تھے تو فارماسی چارجز الگ ہیں الگ ہیں
I am very happy about it and every year ہم progress دیکھ رہے ہیں because اچھے systems لہ رہے ہیں processes implementation کر رہے ہیں باہر سے ہم سیکھ رہے ہیں so the major difference is American system or western system بہت mature system ہے اس کا مطلب کیا ہے اس کے سسٹم میں everything is done you know with well established processes planning okay okay وہ plan سے جب patient hospital کو آتا ہے so ایک system ہوتا ہے ان کے insurances وغیرہ ہوتے verification ہوتا ہے ان سے communication کیسا جاری رکھنا کیسا بتانا ان لوگ کو کیا ہو رہا ہے regularly update کرنا اور کہاں ہیرا فیری نہیں ہو سکتا ہے وہاں پر because کوئی ان کا بل بر رہے ہیں تو ان کا ذمہ داری نہیں ہے وہاں پر پیسوں کے بارے میں نہیں سونستا ہے وہ ان کے جیب سے نہیں آ رہا ہے اس لئے ہیلتھ کیر میں جب تک ایک تھرڈ پارٹی سسٹم نہ ہو verification yeah verification تھرڈ پارٹی سسٹم payment نہ ہو means کوئی گورنمنٹ پی کر رہی ہے نہیں تو کوئی آپ کا private organization پی کر رہی ہے you know نہیں تو آپ کچھ کا آپ کا بزنس ہے آپ انشورنس لیے ہیں آپ کے امپلائیز کو انشورنس دیے ہیں as long as a third party system جب ہوتا ہے جب لوگوں کو شانتی ملتا ہے میرے بیمارے کے ساتھ پیسوں کی problem اس کی بھوج میرے اوپر نہیں ہے ہیلتھ کیر میں ہونا چاہیے so American system میں جیدہ تک جن لوگوں کو انشورنس ہوتا ہے یعنی تو federal government وہاں پر federal government جو insurance provide کرتی ہے Medicare 65 سال کے بعد آپ کو Medicare insurance ہوتا ہے اس کے لئے بھی premium بڑھنا پڑتا ہے مگر بہت کم ہوتا ہے so government is taking the guarantee to do the Medicare this thing مگر اتنا کم میں نہیں جب یہاں پر کر رہے ہیں ہم you know اے پی ہو جنا prime minister ساب کا جو ہے وہ بہت کم ہے جو ہم ہلتھ پروائیڈ کر رہے ہیں آگے جا کے وہ بڑے گا I'm sure they will match that and what not مگر وہاں پر federal system is excellent every doctor accepts it majority of the doctors وہ insurance accept کرتے ہیں وہ patients کو دیکھتے ہیں اس لیے ایسے اچھے طرح سے چلتا ہے مگر وہاں کے problems جو ہیں اب میں جو دیکھ رہا ہوں اس لیے میں امریکہ گیا ہوں کی higher education کے لیے میں میں میں مدیکل کالج میں تھا میں super education ہونے چاہیے وہاں پر میں training کروں گا سب ہونے کے بعد اس ستم میں تھا
56:18On log merese aake bhi bolte hai, dr. Reddy, I don't want to go. You know, because my parents are here, my parents here are here, bachyong ko leke jana raha hai, so if my hair is a tanka here, to me, two, three, three, four, three years, we go and earn and then we will go back. Then we can go to your hospital, continental, and we can go back. It is really a good person. It really emotionally pains me. sometimes I used to tell our CFO in long no sir it will disturb the whole already everybody is talking our hospital hospital management but there is one one thing which I have noticed which is broken apart from what you have told me in India's system which is very very broken is many times patients either keep them alive or survive just because they get extra profits when you know that things can go wrong and there are cases that in big hospitals sometimes patient has died also but then still their families and money have you seen any case like that around the country definitely Definitely.
57:47Definitely. See,
57:53which is unnecessarily prolonging to it, is the fact that their disease is a terminal. Okay? That is very wrong. That is a crime. People do it. People do it. Doctors do it. hospital management pressure now look 80 years of patient terminal cancer now family say everything do everything this is a very common word America do everything do everything do everything because there is no money if people understand some people understand some people don't understand chor The doctor , conselt Oh my
59:08First 80 % patients get to get get to to to to get to get to get to be
59:28only as doctors but they should be the guide they should own family to own a family member empathetically they need to communicate to them you know
59:53let him leave knowing fully well he won't leave or patient stroke I have told you that there is a stroke. We have all this stuff. We know that we can't talk about it. We can't talk about it. We have a lung infection. We have a dialysis. Where are we going to take care of it? But one percent wonder miracle is not a chance? Yes. If there is a chance, that's a spiritual chance. I will give you a right example. My mother at age 72 She was the influencer for me to come back She was very much instrumental but I have a fear that my mother can save me. I will give you an example. His heart surgery was done.
1:01:15After his infection was in hospital. He was a better hospital in Zahidabad. It was a very old infrastructure. So, once again. After that I would return to a hospital and discharge it. Then my girlfriend's discharge was discharged. My son's homelessness was found on California, and my husband had relationships in California and I excite.
1:01:43So, I took the sick women's這邊 in California. After that, we canustes just go for them by Nuevo Bactone. But guide me to apply the hospital. heart bypass qualms for hypnosis where after his infection infection after him to Praud cultures as he did 5-6 days then he got after hospital ICU then 2 the the getting in hospital and remembered آگئے آنے کے بعد still as a doctor لاکھوں cases دیکھنے کے بعد میرے زندگی میں میرے کو ایک سمجھتی کہ اگر کوئی magic ہوئیں گا Bhagawan کی قرآن سے میری ماں اٹھیں گی یہ ventilator سے باہر آئیں گی although میرے کو معلوم تھا one thousand percent okay she won't come back so my example say every person has a hope but we should go to understand and understand when doctors tell us okay okay, inka bachna bilkul na munkun hai, nahin ho sakta hai.
1:03:28It's very important to understand that. Do you think India may be because he spoke about death, jiasa UK mein her hospital ko epna mortality rate publish karna hota publicly. Ki humare hospital mein kitni deaths hoi hai, kitne patients aya thai aur phir kitni deaths hoi, woh public data her saal unko update karna hota hai. Tho sab ko pata hota kohns hospital mein kitni date hoi hai. how many deaths are you? US has public data that the world's lowest mortality rate is in US. India has no system. India has no system. I will tell you. Every hospital is published? No, not her hospital. So, the accreditation is called.
1:04:06Our hospital is so many accreditation. America's Joint Commission. This is a requirement. This is a requirement which every 2-3 years has a survey. is a survey one week has not seen every month we have to record our quality team we should know as a chairman of the hospital if I know this is where the hospital is going to die in which doctor in which specialty is not the calculation is not the statistics How do you really manage? So in good hospitals in this country, definitely that statistical data is a reporting authority in the hospital DME system. Our death is reported to your state's directorate.
1:05:07But it doesn't happen publicly. Now that is important. See, where this should come in India.
1:05:23I would like to write so many good systems I really mean it I'm talking to you These are simple things See, you're already doing But take this to the next level Okay, publish it I hear that there is pressure to not disclose the hospital.
1:05:49They don't want to disclose. Which is true. In some hospitals, but in corporate hospitals, the death rate is so high, they don't want to disclose. So, that's why it's because of influence. The protocol is not made. The policy is not made. but when government mandate can be a regulation now the law is that NABH hospitals can be a little reimbursement now hospitals can be grade it could be a good system this way hospitals you have to submit this data state-wise they can publish that properly classification so we have to report and good hospitals which are accredited data follow quality review if a surgeon has more deaths our disciplinary action lena padata hai and then unko badalna padata hai India mein hota hai magar on a scale that is required compulsorily ho nahi hai that should happen what's mortality rate in your hospital my mortality rate is way below you know not even 1 to 1.5 percent or something like that okay now one thing the other thing is important with the mortality rate what is the type of cases you have agar cancer hospital hai so obviously your mortality rate will be higher accidental trauma is high this is case mix index case mix index what is a hospital we have multi special we have prevention transplant so there is a blended rate so then again but each section we have mortality data what what is is what what Oncology Me Kya Kya Kya Kya Kya Kya Kya Every Minute Detail Hemaare Awards We Present Our Data Trauma Data I Am A Nut Case I Am Data Driven And Doctors Should Be Informed Entire Staff Should Be Informed I Don't Like Anything Hidden Agenda Nice But Then I Am Just Thinking Out Loud If mortality rate which is a great thing it can also be a bad thing backfire in a way that if hospitals know that from mortality rates maybe their ratings affect or public data affect so critical cases where they know they won't even try because we don't want that mark on ourselves mortality rate which is a hospital which you know which you have to tell about America and UK.
1:09:00You have to benchmark. Okay. So the government accepts a benchmarking index. See in America benchmarking index is developed by for example say Mayo Clinic benchmark, Harvard benchmark. And it's CDC. Every thing we get there is CDC Center for Disease Control. we compare here we compare in Hindustan so that our organizations NA, BHA I will tell them many times you have done so good and you have done why don't you publish benchmarking data so that we can benchmark so when the hospital is below benchmark then there is no problem what's the responsibility of a hospital if someone has died do you think hospitals doctors should have responsibility or accountability or no it's a I'm just thinking out loud and asking you this question something should have responsibility for doctors or have they died or have they promised or not promised they can try their best they can try to save their own but it's been a reason what's the procedure what do hospitals do Raj ji, heart death responsibility heart doctor go on a expected death unexpected death the doctor should have 1000 % responsibility if the doctor heart attack doctor's emergency cath lab go there straight work stent ڈالے ہیں stent ڈالنے سے bar ICU میں admit کرے پھر patient مر گیا but this is a recognizable complication means stent ڈالنے سے 1 % 2 % 3 % لوگوں کو stent thrombus جو blood clot ہو سکتا ہے اس کے وجہ سے sudden cardiac death ہونے کا آوشکتہ ہوتی ہے اچھے doctor think the republican these expected complications jab otan hai is liye hum patient se consent mangtay hai, sign kar watay hai ye ho saktah hai magar jab onhe ke baad a good doctor always feels aray 40 saal ka umar, jawan ladka ya admi joh hospital ko aya bichara, first time severe heart attacks hai my cheek tari se time mein aya ho unko cat lab le chala ho jho karna hai mein teek tari se kara ho mayra experience hai mayra naam hai society mein sab kuch hai magar a complication hoa a doctor karne se complication nahi hai this is one of the unexpected complications that happens but he has to still take the responsibility so hum kya karate hai achche hospitals mein mortality morbidity committee hota hai this mortality, morbidity committee every patient death of that month is reviewed this committee 8-10 doctors are chair person every 2 years we change that chair person so so poor deaths 2 weeks before meeting chair person then two committee members choose to choose all review which list and which not and after that the committee will the doctor has to explain their cross-questioning is grading 0 to 4 that grading paper goes to credentialing file it's a must and doctors full prepare کر کے آتے ہیں powerpoint presentation کے ساتھ cross questioning ہوتا ہے ان کو جیسا judge ساپ you know let's say jury بیٹھی ہوئے so deaths are not taken lightly no no all unexpected deaths expected deaths جب dnr fumble کے رہتا ہیں ان کو intubation نہیں کرنا ventilation نہیں کرنا وہ ایسے deaths کو review کرنے کے ضرورت نہیں مگر expected unexpected deaths ہوتے ہیں not only that complications applications.
1:13:44If a patient's surgery is done repeatedly 24 hours, then operation theater is done. All those are also reviewed. OT committee has reviewed and then mortality and mobility committee. It's a very powerful committee. Hospital has three committees very powerful have worked. They have to function to do quality. Second committee is infection control. Very important. Third is mortality and morbidity. Tell me 25-30 % hospitals What's the dirtiest secret about Indian hospitals which public doesn't know The dirtiest secret is our hospitals infrastructure is one of the biggest common denominator infrastructure means that hospitals buildings where the hospital is where is it?
1:14:48Where is the hospital? There is a converted building or a converted hotel. It is established in the hospital. In the hospital, the circulation of the wind is where is the ventilation, the sun is where is the lobby system, the lift system. कि इंदस्तान में one of the biggest causes of death is infections. So getting infections is very easy. Secondary, tertiary infections जजाकर लोग मर जाते हैं, it's inexcusable. बिचारा आ गया सोता, the doctors already were sick and you know what can we do fundamental problem you can see one building is a big sign is a super specialty hospital ok and they say there are 200 beds and the whole what is it is 30 ,000 or 40 or 25 ,000 square foot now see our continental hospital 1.3 million square foot hospital is 18 floors okay well 600 beds per room average square foot is 1300 1350 why do we have this space because we followed healthcare infrastructure guidelines and some people say oh you know hospital this is not for me I can invest in real estate you know you know God has been great to me I have been very successful but I didn't want to be that even till today when people ask me no this is what I dedicated for so I came back if I I want to do this in my terms.
1:17:06I am not going to violate any principle of healthcare infrastructure. Air circulation in OTI theaters, ICU, positive preservation, negative preservation, sunlight. There is no room in continental where sunlight doesn't exist. See, ventilation के बारे में, light के बारे में you know जो हमारे nursing की founder, leader Florence Nightingale वो बताई थी की 18th, 19th century में की 60 % तक हर मरीज का disease अच्छी light होने से अच्छी हवा होने se, achha nutrition hones se, infection control practices hones se wo bimari kam ho jayenga aaj tak be 21st century mein sahih bhaat hai true, they go when the air circulation is not good, okay people are coughing, people have infections, cross contamination ekka bimari, duus raga bimari, threes raga bimari you know you come with one infection, you're getting another infection, you're prolonging the stay you know 65 % there was a statistic 2 years ago paper in public shot Hindustan people who go to regular rooms they go to many hospitals in ICU if you have urinary tract infection it has pneumonia if you have pneumonia it has UTI it has ventilation this is not because doctors' negligence you can understand because if you have permission to get people who is the responsibility of people?
1:18:57See, licensing which is what is happening when the methodology is happening is the right way. It's a very simple process of intervention. You're saving the public. So we're making our own policies because of hydrogenesis. Absolutely. We're getting very happy that we're giving them a cowl, a dals, the health system is so bad.
1:19:26Talking about healthcare system and doctors. I have a very important question.
1:19:37RMPs. Registered Medical Practitioners. Okay. Post-Independence India. When our doctors were very low. So registered medical practitioners. So, they gave them a certificate. They went to the course of 6 months, and they went to the course of 6 months, and they went to different states, different districts. They banned them in our Telangani. RMPs cannot practice. And RMPs practice was very hard. They were more than the people who you know where are you where are you where are you where are you so steroids gave them patients diabetes antibodies every one can be penicillin injection and some people have 100 ,000 ,000 rupees and it's okay in a village when priests 1000 before patients saw the temple temple ke pujari or ne to community ke koji bada admi joh hai you know unko kuch malum hai joh ke tohra bot educated hai jhaad ke pate uhtte uhtte sab leke you know uska paste karke or zukam ke liye ye kama hata hai is ke liye ye kama hata hai the same thing is continuing these registered medical practitioners some states now illegal ban kari they were definitely the cause their doctors about leader in their cities who have under rip bier
1:21:24first line up would do would do doctor would call sister étcape swab since theрьева states for of 5-6 year government health care insurance 1000 uteruses Nikal diya women se Ek doctor Ek rural district Is lake government Par surgery 20 ,000 25 ,000 Deh ra ha Hysterectomize Kar viyay 25 Saal ke Larkiyon Me 30 Saal ke Larkiyon Me You know See These are the things So much happens This is all Because again Nexus See Agar Yeh Nexus where people are seeing if they are finished then there are no things that are not happening and organ is that a black market is that a nexus yes a very good question I will give you a few minutes to tell you it's very important for our country I am very emotional in this topic this is why I am emotional see organ donation itna important it is to be able to you know two organs one is kidney transplantation and the other is liver these two organs it is so much need okay Hindustan in India there are a few more than 2-3 million people every year kidney organs need which is the number of surgery Transplantation 12 ,400 12 ,500 That's about it And liver transplantation 4000 And the other 50 ,000 people Which have no need to deliver Organs So I have to ask When I came to America What problem is In our Hindustan So so my doctor policy leader so 1994 when the transplantation act is transplantation of human organ act is the act 1994 act after amendments came in the act in a thing was commercial commercialization of organ transplantations should never happen.
1:24:12This is why we did that. There were many kidney scandals in the 1970s and 80s. People were buying kidneys for the money. They were going to go out. The Middle East, Sri Lanka, other countries. go to the hospital. So, we should stop this transplantation. We should be strict. That we should take the transplantation to our home. That means blood related people. Okay. Now, let me tell you this. This is government's problem. Organized criminal gangs or criminal way
1:25:01To finish that.
1:25:28This are the European countries 네 the most wealthy countries, wined those countries in the eastern coast of SDC2-n USA, theavalabile officers are referring to these countries that there are people justMB Artist 3 在 muscle area that you will come, any family member is, or your next door, your neighbor is your neighbor, or your friend, you can give them the money. You can give them the money. Your government means, not an insurance company, not yourself personally. Only to that extent. Means, whatever costs are, and not only that, Spain, countries and UK countries after that 3 months until the donor is the donor is the government responsibility is how good system is here in India 80 % live transplants are only 20 % cadaver transplants this is reverse but in the country donor you know cadaver donor transplants ka 87 % life transplants 20-30 because they come to donate but like death after organ transplant it can be used that's not allowed that's allowed no it is allowed but the problem is people don't give them.
1:27:18Why don't they give them? They don't want. That's the biggest problem. But if I die. See, unless you, your family member or unless you have registered if you have a donor registration. If at the time of registration a question is, would you like to donate your organs? You can change later on if you want to. If it's okay, so by garlic something happened to you so you have to get into hospital then you have to give the organ so we verify it digitally this is the consent when someone can't hold it but it's not our system so family members decide so 90 % family members they don't want it and we can't take it basically neurological cases okay usually traffic accidents where the kidneys are preserved liver is preserved we can take it but there is a good education in smaller hospitals education organs harvest and take it in big hospitals this is the problem such a big population will be taken from God son body son body
1:29:07and sk he a Cornias can give a heart. Two plus one, three plus one, four plus two, six. Six patients, six marids who you have a new life and you are going to go. What is the problem? Okay, okay, this is the number of But, live donation is the Why restrict it in India? Why should only be the blood donors? Why don't you allow extended family why laws are all made and sit down developed countries implement so well that they say that racket will go racket will go no they say that you are the neighbor who is going to buy from the price so my that now it goes it goes it goes it goes it goes but there is big organized
1:30:12But problem is whose? Why don't you relax it? Why don't you completely relax it and learn from the other countries that they've implemented so well? A system in Spain or Western countries, opt-in or opt-out. Opt-in means that every citizen of Spain is automatically registered. for organ transplant organ automatically but you can opt out the opt-in is if you don't opt out so you're considered automatically your government your organ can be taken that's good so lot of countries now they are utilizing western nations opt-in and opt-out opt-out it when you have consent you don't have the organ but who is opt-in hai, you know, and two countries have made compulsory.
1:31:09Ek Sweden. Okay, I think even Germany. So, compulsory karne se, they were able to raise 600 ,000 transplants. Kahan 12 ,000. Kahan 4 ,000. Kahan 6 ,000. Kahan 140 crores. And Kahan 600 ,000 such a small country people are ready to transplant. a policy problem we cannot basically say I'm telling you if we go through this you know alcoholism is increasing liver disease is increasing okay almost 20 lakh people die from liver cirrhosis in our country our total death rate is 1 crore you know something like that you know 15 to 20 lakhs people who digest from liver disease okay you know not only from alcohol but other causes of liver disease when they have one girl they have their conditions they have much bigger than they have much fat or diabetes so in some families there is no ideal transplant donor so they can be a first cousin can be possible.
1:32:27We have to go to government, department profile submit, secretary, then we say that certificate is not necessary, pay attention establishes. Okay. We have to doubt. Then we have to go back to committee. We have to go back to two months, three months, and then we have to go back. Then we have to go back. and then he is in the God's hand. The other thing is, if your good friend is ahead, if your whole genetic compatibility is, why not give it? Why should not like other countries? Yeah, why should not? Why should you have the suspicion that you should have, this is not for sale, nothing, there is no financial transaction, he is a real friend, then what will happen?
1:33:23so this is a big problem you know I think itna achcha hum forward policies kare humare desh mein you know agar humare government health is a state subject correct but I say subjects of transplantation also is a big central subject you have to national subject you know very easily you can change people beg cry young children 18-20 losing their fathers losing their mothers 40-45 you know and in cadaver listing when comes to cadaver if it comes to one month or one year two years Northern India is worse in cadaver transplants Southern India is better number one state in cadaver transplants always led is Tamil Nadu and Telangana Telangana last year was voted number one in highest cadaver transplantation in 2024 the state that I am in right now in Hyderabad so I am proud of that tell me the other side of it do you sell organs?
Read the full transcript
1:34:30they must sell but if the racket is bust what is the most expensive organ which gets sold in the market? mostly it is kidneys liver cannot sell liver is very sick the donor has to it's a very big process to basically take it out and all of those things. Kidney is a big thing. Kidney is organs. You know, there was a big racket. Kuch saal peale bangladesh hi. Rackets all connection. Okay. Ouh Singapur me bhi wa ta. Hindustan mein kahi states mein hoa so hai. You know. That was a West Bengal ki border hai jahan pe hai. There was a big, big, big racket.
1:35:16Right. Right. so under the radar I think something can happen but the police is very vigilant and the extortion kidnapping and that was coming out do you know about all of this I think there were some Bangladesh Nepal West Bengal parts of it right they were kidnapping random kids and then they would take out kidneys and then they exactly exactly now it was come over you kidnap them they are they are 24 hours kidney nikal dana kali ono utnane ke baad kali scars dikta hai kudu pata mga ab nahi ho raha hai makar pockets mein wotah hai how much is a kidney priced in the transplantation prices you know 8-9 lakh say 10-12 lakhs and if somebody wants to buy it on a black market which is racket ke through bikra oh mujhe pata nahi i don't even smell what they think but paper ma joh pardet hain ek pachas thousand, one, two, one people are selling their kidneys for that a profitable hospital start how much capital needs to be and what nuances do you think tell me if I want to start a hospital tomorrow how much money needs to be so now the language you will tell us about hospital hotel industry hospital industry hospital industry we calculate per room of charges.
1:36:48In the 21st century, we have a good hospital to make a good hospital. So, the hospital cost is 80 ,000 ,000 ,000. It's 1.5 crores per bed. That's how you calculate if you have 200 bed, 300 bed, 400 bed. Now, there are some differences. Where are you? Tier 1, Tier 2. remember because land cost. India one variable cost is a land cost. Western countries land cost not cost of construction both America 1 million per bed today. 1 million to 1.2 million. This is the story of the story. Because wherever you can make not only hospitals, you can make a hotel even in a tenth of a percent we can build here India has almost 1 crore per bed so India has 1 crore 80 crore, an excellent hospital no, a mediocre hospital middle level is 40 lakhs 60 lakhs, but that's what we can do but the whole every details architecture recommendations this is the current day cost if you build a 600 bed hospital bana na toh 600, 500, 600 crore investment.
1:38:10Yeah, that's the investment. Now this is sometimes including equipment or sometimes without including its equipment. Sometimes kya specialty say hospital mein? Just humare hospital mein 55 specialties hai. Okay, so obviously 55 specialties ki technology, equipment, instrumentation, you know, operation theatres, you know, 70 se leke asit tak, 100 tak, you know, ICU beds this is one broad parameter and every year working capital of this scale hospitals will not make money until 4 to 5 years break even but for working capital the working capital now you basically mark 3 months of whatever is your overhead expense and your costs of running the hospital so for example let's say one crore is per room per bedroom for one hospital room you up 200 room کا binaatے ہو to 200 crore के aspas moita moita investment up per room cost kitna होता है चलाने के लिए good hospitals yeah per room again usme mix होता है राज एक private होता है semi private होता है deluxe होता है so sweets के साफ से होता है अगर private room है usually the rates are 4000 or 5000 or 6000 in that category or whatever the rates are.
1:39:40Hospital business if you look at it it's truly a very very difficult business. Okay? I'll take this challenge in any debate. It's not an easy business. Business business side say kbhi hospital mad banao okay for example 200 bedroom 200 crore you have invested and 4th or 5th year break even capex 4th and 5th year you should break even no break even you are not putting any more working capital to run the hospital whatever the revenues you are making whatever the revenues you are making from the sell sustainability profits have been going on and going on and going on and going on and going on and going on and going on and going on so this is when this is 200 when comes back in 10 years or 12 years 8 to 10 years or 12 years see if you alone have been in our country interest rates compared to western countries have been than in almost 8 9 10 years you'll start seeing some profits flowing.
1:41:01Then there is a dollar-rupee exchange. If you look at me, when I bought land, I was a dollar-rupee, 35 rupees, 37 rupees. Now, if you understand, today, 86 rupees, give me the profit as much as I get. Okay? What will I get? It is washed out in the exchange. Yeah. Correct? No. Two and a half times, my dollar, my money, is got depreciated at the end of the day. Many people say that in Hyderabad, we are in a very beautiful place. You should visit. You should visit. We are in IT and financial districts. Western part of Hyderabad. The most beautiful part of Hyderabad. I am going to sell my land. I have been sold in Manhattan in 2008.
1:41:53Because I was strong. I am going to build corporate hospitals. I want to really do a good hospital in a nice area where people don't have access People have access easy. Parking facilities are. Ventilation, all these things are. When I was thinking about it, land prices were so high. Hindustan was in a way. It was in a corona when I was buying. And then when I was thinking about it, when land prices were so high, which were
1:42:37I would have been today, you know, hundreds of crores I would have, I would have earned. But I didn't come to be a real estate person. Let me keep on reinvesting to make this best and what I can do for my patients. that's where I stick to the line and this is you can verify this also 1000 % tell me in a hospital business where the cost and where the profit is? the profit that you have the patient when you have diagnosed it's treatment in the shortest stay then the hospital becomes profitable we call it length of stay say it. Okay. Length of stay of the length is very dependent. This is why not only for the patient but for the hospital too.
1:43:34So, continental there is a rule that today the patient admits my doctor will give me this indication or responsibility that you should diagnose 24 hours and which is possible. Okay. How can it happen? If the patient is admitted to the patient, it is a good one. Two months ago, it is a good one.
1:44:05Primary care, we have admitted to the doctor. The doctor's name is Dr. Roberts. Now Roberts has the patient to admit. Now Roberts is internist, very good, internal medicine doctor. He has done blood cultures, کر لیے مگر انہوں immediately pulmonologist lung specialist کو اسی دن بھولانا چاہیے انہوں آکے دیکھنا چاہیے اگر ایک doctor کی کچھ kidney کی problem ہے وہی دن جو admit اگر ایسا کرے تو 24 hours میں پورے consultants آکے وہ patient کے بارے میں ذکر کر کے diagnosis کر کے ان کو discharge کر سکتے ہیں the problem is وہ نہیں ہوتا ہے آج ہے doctor Robert صاحب دیکھیں کل Kumar صاحب آ رہے ہیں now Kumar Sahib consultation he came to I was very busy in OP he was 4 in the evening then he was next day he was CT of the chest he was then the third day Kumar Sahib came to the lunch he said he said I will bronchoscopy then he bronchoscopy after he bronchoscopy after he he was doing he was waiting for 2 days he was this is what happens efficiencies of scale in every discipline and profession and life is extremely important I'm sure you're extraordinarily efficient you know without that it's not possible and what you've achieved is not possible so efficiencies in healthcare it is a quality parameter the word efficiency infrastructure you know efficiency is not to make money efficiency is the person joh sick hai joh mareez joh problem se aar hai kitna jildi se aap uski diagnosis karke sain diagnosis karke agar aap treatment shuru kar sake to that is efficiency and that's what is a profit maker that was a profit because unnecessarily you're not tying up and losses and not only that you are basically you know insurance company only it will pay you this much all the rest you have to absorb and sab se bada cost center transplantations IS cost centers transplantations bone marrow transplantations you know it's more expensive you don't end up making money transplant yes transplantations margin of profit so see good hospitals will never make more than you know at best single digits to double digits 10, 15, 16 percent yes you know that too after this end of maturity so if you annualize over 20 years from start to the break even time to making profits it will always be in single digits but that nakshah is changing to some extent which I am concerned what is happening the investments in hospitals the change is growing the millions of dollars millions of dollars invests are doing okay it is okay Hindustan is needed but what is happening what is happening what is happening that you don't necessarily you know cost increase what do you think about this cost ultimately absorb because they know private equity because you know 4 years, 3 years 5 years exit strategy exit strategy then they will send other private equity so this racket this system now It's starting.
1:47:47So I applaud investments. But also do something for the country. Only purpose is not bottom line. You're coming from industrialized nations, best policies. Bring them in. Have a human resource transfer. Have your technology transfers. We don't need money for technology. We have better CT machines and MRIs than most countries in the world. okay but the problem is that the areas where healthcare management healthcare leadership is a huge debt it's very low I say focus on that bring this to the next level cost efficiencies you bring in not this hospital one doctor to take this one to 10-12 million another I'll go to my hospital and sit in the boardroom and I'll go to my five doctors where did I go to my back?
1:48:42I'll go to my back you know this is what is happening I'm sorry to say the entrepreneur in you came out right now I could see that pain but thank you so much for spending time with me sir thank you it was lovely getting to know about what are the wrong things which are going on in our country in the healthcare space and how you guys are addressing it thank you so much I had way more questions but then I think you're short on time so I have plenty of time I'll do part two okay lovely we'll do then we'll do health concerns there you go absolutely because I had a question on burping and that caused me let's go with it we'll do that thank you so much it was a pleasure having you thank you thank you very much I'm not a profession usually I'm not even Telugu podcasters I'm not that enthusiastic but all my digital team my branding team and they were after me sir if he gives you you should do it you should do it with Mr.

In today’s episode, we have Dr. Guru N Reddy, Founder & Chairman of Continental Hospitals. With decades of experience in healthcare across India and the US, he gives an inside look into the business of hospitals, the reality of billing practices, and how patients are often charged far beyond what’s necessary. He also shares his perspective on how India’s healthcare system compares with the corporatised model of America.
He also opens up about the challenges hospitals face, the lack of infrastructure, and the black market of organs. In the last segment, he discusses what it really takes to start and run a hospital, from capital per bed and land costs to annual working expenses, and reveals where hospitals actually make money versus where they incur the most losses.
